Why We Choose Microneedling Over Heat-Based Treatments - Every Time

With the growing popularity of radiofrequency (RF) and laser devices, it’s easy to get caught up in the hype. But the truth is, heat and health don’t go hand in hand. Let’s talk about why we choose to support your skin through the regenerative power of microneedling.
Microneedling creates controlled, microscopic channels in the skin. Think of it like a needle & thread going into a sweater – the needle doesn’t cause a deficit, rather it just pushes the fibres aside to bring the thread (repair) into the garment. This gentle mechanical stimulation activates your body’s natural wound-healing response, the release of growth factors and fibroblast activity that produce healthy, functional collagen and elastin.
On the contrary, when we introduce heat to the skin, the type of collagen our skin produces is “scar collagen” which is much more densely packed and forms in parallel bundles. This is why we think the skin is firmer, when in reality, it’s just scar tissue beneath the surface. Think about a scar from a cut – the skin is tighter where the scar is – it’s the same principal. We also know that scar tissue is much more susceptible to UV damage down the road.
What Heat Really Does Beneath the Surface
Most heat-based treatments like RF microneedling and fractional lasers work by raising skin temperature to between 60–75°C which short-circuits the wound-healing process by coagulating tissue. At this temperature the result is rapid and irreversible denaturation of proteins (like collagen & elastin), destruction of the dermal matrix – beyond repair, leading to fibrosis, textural irregularities and long-term sensitivity.
Denatured proteins coagulate. Think of the way an egg white turns solid when cooked. This is what I mean when I say you are literally cooking your collagen.
Cell membranes break down, lipids melt and enzymes spill out triggering cellular necrosis (ie. cell death). You’re not “stimulating cells”, you’re killing them, intentionally, in a patterned way.
Collagen fibers shrink and lose their natural flexibility. This causes the “immediate tightening” effect seen after RF treatments, however, denatured collagen is non-functional; the body must later replace it.
Sebaceous and sweat glands become permanently damaged (this is why clients see a reduction in oil production post treatment).
The short-term result? A tighter, smoother appearance.
The long-term outcome? Often chronic dryness, compromised barrier function, hyperpigmentation, and structural stiffness — signs that the skin has repaired through scarring, not regeneration. The sebaceous glands, in particular, play a key role in maintaining the skin’s microbiome and lipid barrier. When they’re thermally injured, clients often experience persistent dehydration and dullness and even long- term loss of elasticity over time. Clients who receive repeated laser and RF treatments (and sometimes even repeated Botox injections) eventually develop a “stiff,” waxy texture, often mistaken for “tight” skin (and this is just the tip of the iceberg).
Yes, a big part of the “sebum reduction” is literally sebaceous gland damage / destruction. That’s not a side effect – it’s the goal in med-based acne protocols. The science is pretty blunt about this: RF causes the glands to be destroyed or shrunken and then replaced by fibrous tissue, which also tightens the dermis and reduces pore size. Is that “wrong” in a conventional derm sense? Not necessarily – if someone has severe, acne and is choosing between Accutane vs. localized partial gland ablation, some dermatologists see that as a fair trade. But from a skin-health perspective, sebaceous glands are an important part of a healthy barrier & microbiome, not just oil factories to be nuked. Replacing functioning glands with fibrous tissue is not the same as restoring balanced, resilient skin; it’s symptom suppression via controlled destruction.
Histologically, RF microneedling clearly produces columns of coagulated tissue in the dermis, followed by replacement with fibrous tissue. Many practitioners call that “dermal remodeling” and present it as a benefit, particularly for those with acne scarring. But, from a purely functional lens, you can absolutely argue that if we are repeatedly creating deep coagulated columns that heal with fibrous tissue, we are by definition increasing localized scar tissue in the dermis, even if the surface looks smoother. It’s just not something most aesthetic studies are designed to measure or admit. They’re grading photos and short-term texture, not long-term tissue quality. This caution isn’t anti-science; it’s reading the mechanism with a more long-view than most device brochures.
These temperatures are not “bio-stimulating”, they are ablative and destructive, relying on the body’s emergency repair system to remodel the area. Traditional microneedling, by contrast, works through mechanical micro-injury that preserves cellular viability and vascular supply, keeping the regeneration process cleaner and less fibrotic.
The truth is: the long-term damage from RF microneedling can take months to years to fully reveal itself, because of how slow the skin remodels beneath the surface. Fibrosis, fat loss, dehydration, dullness, and loss of elasticity develop gradually and are easy to misinterpret at first as normal aging or “collagen turnover.” Most before/after photos are taken at 1–3 months, when inflammation still masks the damage.
But we believe your skin deserves better, and we also know that trials have shown traditional microneedling to produce better outcomes for pore reduction and skin brightening. Systematic reviews have shown no statistically significant difference in aesthetic outcomes to support the increased cost (as much as double or triple a trad MN session) and risk that is associated with heat-based technologies. The “faster, better, fewer sessions” claims are appealing, but they’re just not backed up by the data.
When practitioners understand skin only as layers to treat - epidermis, dermis, subcutaneous - it’s easy to believe that heat-induced “collagen remodeling” is a positive thing. But when we understand cellular biology, we realize every fibroblast, keratinocyte, and endothelial cell has a temperature tolerance and beyond that threshold, the cell dies, not “activates.” What follows isn’t regeneration, it’s a wound-repair cascade that prioritizes speed over integrity, and the result is fibrotic collagen (scar tissue), not functional collagen. When you understand how cells live, communicate, and die, you stop celebrating destruction as rejuvenation. You start honouring the skin as a living organ, not a canvas to resurface.
